Here, in the better places, they tend to use what we call doonas. They don't put the quilts inside the dooas. Rather they have bottom sheet, then top, then doona, then doona cover. I imagine the actually doona is not washed each time. Hopefully all sheets are. (DUVET...just looked it up).

I read somewhere about bed linen having a tag that tells when they were last washed.
